Medicare Annual Wellness Visits

These yearly visits help Medicare beneficiaries get a health risk assessment (HRA) and create or update a personalized plan to address gaps in care, target disease prevention and promote a holistic health experience. Medicare Annual Wellness Visits are designed to give you a platform to address all your health needs, not just current symptoms. The ultimate goal of completing a Medicare Annual Wellness Visit is to equip your provider with a detailed understanding of how best to support and care for you, and to help you get more involved in your health outcomes.

How it Works

Step 1

To schedule a Medicare Annual Wellness Visit, navigate to our scheduler and select “Medicare Annual Wellness Visits” as your appointment type.

Step 2

Our health care providers will walk you through a health risk assessment (HRA) – a series of questions to help us learn more about you as a unique patient.

Step 3

Following the assessment, we’ll review the results of your questionnaire, identify gaps in care and assist in developing a plan to address those gaps.

Step 4

Several gaps in care can be addressed inside The Little Clinic. We can also collaborate with your primary care provider (PCP) to deliver follow-up care.
